The Problem

Complexity of Schizophrenia Care & Relapse Risk

Schizophrenia and related psychosis-spectrum disorders are complex, chronic conditions that require ongoing monitoring, adherence, and coordinated care.

Challenges include:

  • Low medication adherence (less than 50%)
  • High levels of social isolation and reduced daily functioning
  • Cognitive impairments impacting self-management
  • High readmission rates (23% within 30 days)

These challenges contribute to significant system burden, including an estimated $6.9 billion in healthcare costs and lost productivity.

The challenge is enabling continuous, personalized support to reduce relapse risk and improve long-term functioning.

What is A4i?

Mobile application and clinical portal for severe mental illness

A4i (App4Independence) is a software as a medical device (SaMD) and digital therapeutic designed to support individuals living with schizophrenia and related psychosis-spectrum conditions.

A4i is a remote monitoring, engagement, and care coordination platform that connects patients and clinicians through a personalized mobile app and clinical dashboard.

The platform uses advanced data signals and machine learning to support:

  • Symptom monitoring and relapse risk prediction
  • Medication adherence and care coordination
  • Personalized engagement through notifications, messaging, and content feeds

A4i includes a patent-pending auditory hallucination support tool, helping users distinguish between environmental sounds and hallucinations, while supporting development of compensatory cognitive skills.

It also incorporates:

  • Relapse risk prediction using behavioral and digital signals
    • Leveraging indicators such as sleep patterns, engagement data, and user input to identify early signs of deterioration
  • Cognitive skill-building through interactive features
    • Supporting development of coping strategies and improved daily functioning

Personalization is continuously refined using:

  • User responses to in-app assessments
  • Behavioral signals (e.g., engagement patterns, phone usage)
  • Sleep-related indicators derived from device activity

A4i delivers a data-driven, adaptive approach to supporting recovery and daily functioning.

Outcomes

High engagement with real outcomes and scalable Impact.

A4i has demonstrated:

  • High user engagement and retention
    • 100% engagement at 7 days and 94% retention at 20 days
  • Improved recovery-related outcomes
    • Users showed improvements in areas such as hope, illness knowledge, and self-efficacy
  • Strong engagement among higher-need users
    • Individuals with more significant symptoms at baseline demonstrated higher usage
  • Support for relapse risk identification
    • Data-driven monitoring enables early identification of potential deterioration
  • Scalable, clinically integrated digital therapeutic approach
    • Designed for deployment across health systems to support ongoing care and coordination
