The issue of patient adherence is complex and involves interacting dimensions of patient-related factors, social/economic factors, condition-related factors, therapy related factors, and health system/healthcare team factors1. In other words, the nature of the adherence beast is multi-faceted and inherently interdisciplinary. Expert advice informing best practices to improve
patient adherence is needed from healthcare-related clinicians (doctors, nurses, pharmacists), mental health experts (psychologists, psychiatrists), sociologists, social workers, epidemiologists, health-related researchers, and policy-makers (among others). A more appropriate term for how adherence should be properly viewed then may be transdiciplinary. That is, adherence requires a holistic approach, one that seeks to integrate knowledge from all related disciplines into a coherent whole.

Will Arizona’s Medicaid beneficiaries use smartphone apps?

Arizona Governor Doug Ducey carved out a place for mobile health in his plan for the state’s new Medicaid plan this week, but some critics think the initiative could miss the mark for a low-income population less likely to have smartphones.

Arizona’s Medicaid system, the Arizona Healthcare Cost Containment System (AHCCCS), is a managed care system that operates on a special waiver from the federal government. That waiver is set to expire next September, and Ducey is using that expiration as an opportunity to make changes to the system that he hopes will cut costs by empowering Medicaid users and helping them get back to work. The changes, a plan called “Choice, Accountability, Responsibility and Engagement” (CARE), include health savings accounts, strategic co-pays, and wellness incentives for Medicaid enrollees.

DC Medicaid payor will offer med adherence texting platform for asthma patients

A new text messaging software aimed at improving medication adherence will be offered to asthma patients who are members of AmeriHealth District of Columbia, Washington, D.C.’s largest Medicaid managed care organization. The platform, called HealthNHand, was developed by Memotext and will be launched by pharmacy benefit management company PerformRx.

HealthNHand will use interactive voice responses, email, and text messages to remind asthma patients in the program to take their medication and to fill out questionnaires about their health. The service will use data from the questionnaires to craft personalized responses to patients about their condition, including information, support, and air quality forecasts.

MaRS’ new District 20 means business

I’m sitting across from Eric Riz, founder of Empty Cubicle, in the boardroom of MaRS’ new co-working space, District 20. A 6,000 square foot section of the MaRS Heritage Building (in what I’m told is rumoured to be the old Toronto General morgue), District 20 blends 20th century facilities with 21st century function in a way most have come to expect from the innovation hub. The sandy-bricked space is filled with colour-coded, pod-like offices; minimalist yet comfortable furniture lines the hallways and common areas, which feature private booths for phone calls or a quick break-out meeting. The space feels warm and professional.

But Riz isn’t talking about the space as an office, or at least not entirely. He’s talking about it as an extension of MaRS. MaRS has a mixed history convincing startups to take residency in its space. Riz is frank in his contrarian opinion.

ABSTRACT

Importance Topical glaucoma medications lower intraocular pressure and alter the course of the disease. Because adherence with glaucoma medications is a known problem, interventions are needed to help those patients who do not take their medications as prescribed.

Objective To assess the ability of an automated telecommunication-based intervention to improve adherence with glaucoma medications.

Design, Setting, and Participants We performed a prospective cohort study of medication adherence, followed by a randomized intervention for those found to be nonadherent, of individuals recruited from a university-based glaucoma subspecialty clinic. A total of 491 participants were enrolled in the initial assessment of adherence. Of those, 70 were nonadherent with their medications after 3 months of electronic monitoring and randomized to intervention and control groups.

Electronic Reminders Can Improve Glaucoma Patients’ Drug Adherence

NEW YORK – A considerable proportion of glaucoma patients don’t take their medicine as prescribed, but electronic monitoring and reminders can boost adherence, according to a pair of reports on a new study.

“We now have two randomized trials — including this one — that have demonstrated an approximately 20% increase in adherence with glaucoma medications in a group shown to be non-adherent,” Dr. Michael Boland, lead author of both papers, told Reuters Health by email.

“There are no other interventions that have shown this level of improvement in this group,” said Dr. Boland from Johns Hopkins University in Baltimore, Maryland.

Both papers were published online May 15 in JAMA Ophthalmology.

How One Healthcare Entrepreneur Created His Own Access

Like many healthcare entrepreneurs, Amos Adler of MEMOTEXT found it difficult to access the stakeholders who could help to make his patient adherence company a success. The challenge, he said, is that access is dependent on how a startup can align with the business model of the stakeholders involved. “Can you get into their business model,” he said, “and can you get into the clinical workflow?”

To get around that, Adler and MEMOTEXT have found their own solution. “We’re in the midst of building out what we call ‘the alliance for adherence,'” he said. The alliance, Adler said, consists of a group of stakeholders who have access to patients and a common interest in patient adherence.

Canadian University Develops New Program To Help Educate Expectant Moms

In this ever-changing world of technology, it is amazing how quickly things change. For example, for a while now, when women were pregnant, they had the option of attending classes focused on prenatal education over the course of their pregnancies. But unfortunately, not everyone can attend these classes due to prior commitments, medical reasons or a variety of other reasons. They would end up missing out on these important educational experiences. But researchers at the University of British Columbia are aiming to change that through the use of educational text messages for these women.

The SmartMom Canada program was created to meet the needs in British Columbia’s northern health region. Researchers’ aim is to have women in that area try out the program, along with women in the Fraser Valley Health region. Patti Janssen, professor at UBC’s School of Population and Public Health is spearheading the initiative, along with the university’s Child and Family Research Institute.

A Spoonful of Sugar: Developing a Medication – Reminder App

Sticking to a health regimen means remembering to take prescribed medications. But studies of medication adherence show that patients often fail to stay with the program. A prescription drug survey published by the National Community Pharmacists Association (NCPA) revealed that nearly 50 percent of consumers said they forgot to take a prescribed medication, while 24 percent said they took less than the recommended dosage.

Automated Med Reminders Boost Adherence

Glaucoma, the world’s second leading cause of blindness, has a number of effective therapies to help those afflicted. Despite these solutions, non-adherence rates for those treatments are reported to be between thirty-seven and fifty-nine percent. Luckily, mobile health professionals are working on ways to solve this issue. One such company, Memotext, has been developing adherence technologies and may have proof that its solution will improve those statistics.

Memotext and Microsoft’s recent study (funded by Microsoft and conducted by Johns Hopkins University) measured the efficacy of using the Memotext adherence solution combined with Microsoft’s personal health tracking platform HealthVault. Patients received some combination of text message reminders and IVR messages to remind them about their medication regimen.
